By paying for services to Jo Ward, NLPLifeCoaching and associated companies/websites I hereby confirm that I am participating voluntarily & agree that all services provided by Jo Ward are done purely to inform. In the event of being in direct participation with Jo Ward, hold her completely harmless, and will seek no compensation or damages for any experience(s) that may occur. I give my consent to therapy being performed on me by Jo Ward.  I understand that there are no guarantees and I hereby give my consent to treatment.

I understand that I receive this treatment at my own risk and I have no claim whatsoever against Jo Ward or NLP Life Coaching for any work that Jo Ward may perform on me or for any incurred personal expense. I acknowledge that Jo Ward is not a Medical Doctor.

I appreciate that, as Jo Ward I have no recourse to her personally or NLP Life Coaching should this work not effect a complete recovery from my particular problem. I accept this event solely seeks to inform & increase awareness, and any changes this may create in me & for me are purely coincidental, I remain totally and personally responsible for everything that affects me & will hold no one else liable.

I am willing to be guided through relaxation, visual imagery, creative visualization, hypnosis, and stress reduction processes and techniques for the purpose of vocational or avocational self-improvement.  I understand that the treatments which may include: Hypnotherapy, Theta Healing, Reiki, NLP, Time Line Therapy, spiritual guidance, Huna, Life Coaching, Wealth Dynamics, Shamanism, Crystals, Colour Therapy and other forms of healing modalities I am receiving is not a substitute for normal medical care and I have been advised to discuss this hypnotherapy with any doctor who is taking care of me now or in the future.  Additionally, I should continue any present medical treatment and consult my regular medical doctor for treatment of any new or old illnesses.
